CMS 1500 Claim Forms

CMS 1500 forms are the standardized medical billing forms for professional (non-institutional) medical billing of health insurance claims in the United States.

Our form is the official CMS 1500 form version is 02/12 in use for non- institutional providers and suppliers to bill Medicare, payers and insurance companies.  No other version of the form  OMB-0938-0999 form is acceptable  to use after April 1, 2014.

Claims must be submitted on original, not photocopied, print versions of the CMS-1500 forms
